Case Management


Medical case management is a collaborative process that includes client centered services facilitating implementation of evidence based clinical treatment plans. This assures that appropriate medical care is provided to disabled, ill or injured individuals while reducing unnecessary utilization.

Case management services links clients with health care, psychosocial, and other services, ensuring timely and coordinated access to medically appropriate levels of health and support services and continuity of care.

Case management includes treatment adherence counseling to ensure readiness for increased compliance with prescribed health care, the delivery of optimum clinical care, and the continual evaluation of client’s and key family members’ needs, and assessment of health outcomes while controlling costs.
